Please complete the following form related to our boys equipment loaner program. You will only need to size the helmet and gloves - the other sizes we will determine based on height and weight.

- For helmets, we use the Cascade CS for grades K through 2 and the Cascade CPVR for all others.
- You do not need to fill out details for a stick. All sticks are one size.
- Instead of filling out sizes for Arm Pads and Shoulder Pads, we will determine size based on height and weight.
